Our satellite office is located at 11546 Chapman Highway, Seymour, TN 37865. Our newest satellite office near the Pellissippi Parkway and Northshore Drive has just opened at 1924 Pinnacle Pointe Way, Suite 200, Knoxville, TN 37922 (located between Kroger and CVS Pharmacy). To help us make your initial visit go smoothly, we ask that you please read and follow the directions below:

1. Please complete the Patient History Form and the Patient Health Survey. It would also be helpful to review our Notice of Privacy Practices.

2. If your insurance plan requires a referral authorization form, it is necessary that you call your primary medical doctor to arrange for them to issue a referral. Without the referral form, your insurance will not cover the cost of this visit. You will then become responsible for all charges. The referral form can be mailed, faxed (865-523-6587), or hand-carried to our office.

3. Many insurance plans require that the patient pay a co-pay for each visit. Payment must be made at the time of the appointment.

4. Bring your medications with you or a list of medications that you are currently taking.

Please plan to arrive 15 minutes prior to your appointment time so that any paperwork may be processed before your visit. We will make every effort for you to be seen at your scheduled time.
